HWCOE Lab Registration Form

Please complete the lab registration form to the best of your ability. This should be a brief overview of the hazards in your lab and take approximately 10 mins to complete.

  • All HWCOE experimental labs must assign a Lab Safety Manager (LSM). If unknown at this time, please enter not assigned.

  • Hazard Analysis

    Please select any hazards associated with the work you intend to conduct in your labs. If you are unsure if any apply, use the text boxes below the options to provide brief details.
  • Please provide a brief description of materials.

  • Briefly indicate UAVs/drones, robotics, 3D printers, Acid digestion, etc. in the text box below.

  • This section is for labs who only conduct computational work and the lab space consists of an office type setting with servers, desks, and chairs.
