Please use the following forms to submit service requests to the Herbert Wertheim College of Engineering Office of Facilities operations. If we are not the correct contact for your request, we will assist you in routing your request to the correct party

College Building Access Request Form

Please use the College Building Access Request Form to request electronic access (Lenel) and/or required building keys.

College Facility Reservation

Please use the College Facility Reservation Form to request the use of one of our special event spaces. Please visit the Facilities Reservation Policies page for more information about our event spaces.

General Services Request Form

Please use the Facilities Operations General Services Request Form for all other requests and inquiries, including, assistance with furniture moves, maintenance, project inquiries, mail questions, etc.

Research Needs Analysis form

The Research Needs Analysis Form is used by Facilities Operations and the Office of Safety to assist new researchers in identifying facility requirements to accommodate research and equipment
