First-year students who wish to change into an engineering major from a non-engineering major should complete this form.

Every student has the right to request an exemption to a policy or deadline, provided there is documentation of an extenuating circumstance.

• Petitions must be submitted online
• They are forwarded to the student’s department for a recommendation
• Petitions will not be reviewed without a departmental recommendation

Use this form to request equivalency for coursework taken at another higher education institution.

Before submitting:

• Check your Degree Audit—the course may already be listed as equivalent.
• If not, submit this form with:
  • The syllabus from the course you completed
  • The name of the UF course you believe it matches

Note: Equivalency is based on content. Transferred credit hours may differ depending on the institution’s credit system.

Faculty or staff may ask students to complete a release form before writing a letter of recommendation.
